When signing an unsigned document (see screenshot), the lower part of the window (under Your review) says

         (someone else is the primary reviewer)

but what causes that, when it was not yet signed by anyone?

Seemingly it bears no relation to who is the primary doctor, because even after I changed the null value to Leonard McCoy it made no difference.
