Some records in the Plucker document are reserved for different kinds
of metadata, e.g. the URL of external links and what category the
document should be put in by default. The index record contains an
array with the name and location of each reserved record.

Note that some features in the 1.27 specification are not supported
by any tool (AFAIK), e.g. Navigation metadata, Sorted URL Handling,
External Anchor Name Handling, and Page List Metadata.
