Hi Jason, the email should not be delivered any times that qmail-scanner
do a temp fail to remote SMTP, but I can see with my eyes the same email
in the user's Maildir/ duplicated every 2-3 minutes.

I also believe that the problem is related to the size of the message,
for what I have noticed every times that the problem occurs the size of
the messagge is around 250K.
In my configuration of qmail-scanner "--sa-maxsize" was not set, so it's
on default value.
I have found two ways to solve the problem:
- one is unapply qmail-scanner (via tcp.smtp) to remote IP from which
the email arrives
- another is set in qmail-scanner "--sa-tempfail 0" and
